Where it came from

Joseph Pilates developed the method in the first half of the twentieth century, originally as rehabilitation. Several pieces of the equipment still in use — the reformer and the Cadillac in particular — descend directly from apparatus he built for bedridden patients using bed springs.

That origin explains a lot about the method. It was designed to let people work meaningfully when they could not support their own body weight, which is why springs assist as often as they resist.

What distinguishes it from other exercise

Precision over volume. A Pilates set is typically five to ten repetitions performed exactly, rather than three sets of twelve performed adequately. The intent is motor control — teaching the body a movement pattern — more than muscular fatigue.

The other distinguishing feature is the equipment. Spring resistance behaves differently from weights: it increases through the range and it can support a movement as well as oppose it.

Mat and equipment

Mat Pilates uses body weight and gravity alone. Equipment Pilates uses the reformer, Cadillac, chair and barrels, all spring-loaded.

Counterintuitively the mat is often harder, because nothing assists you. The reformer has a lower entry point and a higher ceiling.

  • Cadillac — fixed frame with overhead springs, best for rehabilitation
  • Chair — small footprint, high stability demand
  • Barrels — supported spinal extension and side-bending

Who it suits

It is unusually adaptable. The same apparatus serves a post-surgical patient working through tiny ranges and a professional athlete doing loaded standing work, because the spring load and range are so finely adjustable.

That adaptability is the main reason it appears in clinical settings as often as in fitness ones.
