Core strength and trunk endurance improve reliably. Balance and functional mobility improve, particularly in older adults, with fall-prevention programs citing it frequently. Flexibility improves, and the gains hold better than passive stretching because they come with control.

For chronic non-specific low back pain, the evidence is reasonably good — comparable to other well-designed active exercise, which is a meaningful result.

The claims worth ignoring

Pilates does not lengthen muscles. Muscle length is determined by its attachment points, and no exercise changes those. What improves is the range your nervous system permits, which is a real effect described inaccurately.

It also does not "tone without bulk" in any way that differs from other low-load, high-control training. Bulk requires progressive heavy loading and adequate calories; the absence of bulk from Pilates is simply the absence of that stimulus.
